We are in need of an CNA traveler with an ASAP start at our Long Term Care facility, Community Nursing Home of Anaconda. 12 hr shifts; shifts will be 6 a.m.-6:30 p.m. (36 hours/week) scheduled variable days
Compliance Documents CHA requests from agencies:
1. Resume
2. References
3. Employment Verification (at least 3 employment verifications within the past year)
4. Identification
5. Emergency Contact (agency form is acceptable)
6. Proof of COVID vaccination (or declination)
7. Proof of flu shot within last year (or declination)
8. Proof of negative TB test within the last year (we accept skin or blood tests)
9. MMR
10. Varicella
11. Recent physical (within 1 year is fine)
12. Certifications (i.e., BLS, etc.)
13. Recent competencies/compliance training records (what agency has listed in their compliance packet is acceptable)
14. Proof of current licensure/cert
15. Criminal Background Check (standard 7 year)/OIG/SAM
16. Drug screen (5 panel is the minimum we require; 10 panel is acceptable)
17. Signed confidentiality Agreement; acknowledgment via signature/date of exposure control plan; acknowledgement via signature/date of latex allergy
18. Epic Access Request Form (if applicable)
